An Emergency Department (ED) Facility
Coder is responsible for accurately coding medical records and services
provided in an emergency department setting. The role typically involves
reviewing patient records, applying correct diagnosis (ICD-10-CM) and procedure
(CPT, HCPCS) codes, and ensuring compliance with various coding regulations,
payer guidelines, and policies. ED coders work closely with healthcare
providers and billing departments to ensure that all services are captured and
coded correctly for proper reimbursement. Job Responsibilities
1.
Review Patient Records:
Read and interpret medical documentation from emergency department visits,
including physician notes, nursing notes, and diagnostic results.
2.
Code Diagnosis and Procedures:
Assign the appropriate ICD-10-CM diagnosis codes, CPT, and HCPCS codes for
services such as evaluations, emergency treatments, and diagnostic tests.
3.
Apply Modifiers:
Use appropriate modifiers to indicate specific circumstances related to
procedures and services.
4.
Ensure Compliance:
Ensure that coding follows regulatory requirements such as HIPAA, Medicare, and
payer-specific guidelines.
5.
Revenue Cycle Management:
Assist with ensuring accurate billing by providing accurate code sets, which
support claim submission and reimbursement.
6.
Collaboration:
Work with emergency department physicians, billing staff, and management to
clarify information in the records when necessary.
7.
Denial Management:
Review denied claims, make corrections, and resubmit claims to ensure maximum
reimbursement.
8.
Audits and Quality Control:
Participate in internal and external audits to verify accuracy and compliance
with coding standards.
9.
Stay Updated:
Keep up to date with changes in coding guidelines, healthcare regulations, and
reimbursement policies.
Skills and Qualifications

Certification:
Certification such as Certified Professional Coder (CPC), Certified Outpatient
Coder (COC), or Certified Coding Specialist (CCS) is often required.

Medical Terminology:
Strong knowledge of medical terms, anatomy, and procedures related to emergency
care.

Coding Systems:
Proficiency in ICD-10-CM, CPT, HCPCS coding systems.

Attention to Detail:
Accuracy is critical in coding, especially in the fast-paced emergency
department environment.

Analytical Skills:
Ability to analyze medical records and understand the services provided to
ensure correct coding.
