-AA: Anesthesia services performed personally by anesthesiologist: This modifier is used when the anesthesia service is provided by an anesthesiologist. This modifier is typically used when the anesthesiologist is not part of the surgical team and is providing the anesthesia service independently.

Example: A patient undergoes a surgery and the anesthesia service is provided by an anesthesiologist who is not part of the surgical team. In this case, the appropriate modifier to use would be -AA.

 

-QK: Medical direction of two, three or four concurrent anesthesia procedures involving qualified individuals: This modifier is used when an anesthesiologist is directing two, three or four concurrent anesthesia procedures. This modifier is typically used when the anesthesiologist is overseeing the anesthesia services being provided by other qualified individuals.

Example: A patient undergoes a surgery that requires the services of multiple anesthesia providers. In this case, an anesthesiologist is directing the anesthesia services being provided by other qualified individuals. The appropriate modifier to use would be -QK.

 

-QX: CRNA service with medical direction by a physician: This modifier is used when a Certified Registered Nurse Anesthetist (CRNA) is providing anesthesia services under the medical direction of a physician.

Example: A patient undergoes a surgery and the anesthesia service is provided by a CRNA who is working under the medical direction of a physician. In this case, the appropriate modifier to use would be -QX.

 

-QS: Monitored anesthesia care service: This modifier is used when the anesthesia provider is providing monitored anesthesia care (MAC) for a surgical or diagnostic procedure.

Example: A patient undergoes a colonoscopy with MAC provided by an anesthesia provider. In this case, the appropriate modifier to use would be -QS.

 

P1: A normal healthy patient: This modifier is used when the patient does not have any significant medical conditions that would affect the administration of anesthesia.

Example: A patient undergoes a routine dental procedure and is considered to be a normal, healthy patient. In this case, the appropriate modifier to use would be -P1.

 

P2: A patient with mild systemic disease: This modifier is used when the patient has a mild systemic disease that is well-controlled and is not expected to interfere significantly with the administration of anesthesia.

Example: A patient with well-controlled diabetes undergoes a routine surgical procedure. In this case, the appropriate modifier to use would be -P2.

 

P3: A patient with severe systemic disease: This modifier is used when the patient has a severe systemic disease that is poorly controlled and is expected to interfere significantly with the administration of anesthesia.

Example: A patient with severe heart failure undergoes a surgical procedure. In this case, the appropriate modifier to use would be -P3.

 

P4: A patient with severe systemic disease that is a constant threat to life: This modifier is used when the patient has a severe systemic disease that is a constant threat to life and requires the presence of an anesthesia provider throughout the procedure.

Example: A patient with severe uncontrolled hypertension undergoes a surgical procedure. In this case, the appropriate modifier to use would be -P4.
